Initial Observation Report Packet

This Initial Observation Report Packet includes a template, sample report, and an initial observation checklist. These reports communicate results from the Gilliam Autism Rating Scale—Third Edition, the Aberrant Behavior Checklist (ABC-2), and an initial observation checklist. These reports can be individualized for any client and/or clinic and are often paired with ABA Tailor-Made’s Client Intake Packet.

This Initial Observation Sample Summary Report is a model for how therapists and specialized instructors can complete their reports to communicate the information gathered through these tools.

The Initial Observation Summary Report Template allows practitioners to individualize and edit the information so that it is individualized for each client/clinic.

The Observation Checklist allows practitioners to have a guide for initial skills that can be observed in person. Skills from the following domains are included: Social Skills, Communication/Language, Motor Skills, Academics, Visual Performance, and Barriers to performing the observed skills.
